Waiver of Liability
In consideration of being admitted for membership in the Toronto Spartan Volleyball League (TSVL), I the undersigned hereby release and forever discharge the TSVL, its players, captains, officials, coaches, volunteers, and executive members from all liabilities, actions, cause of actions, claims, demands for damages, loss or personal injuries, however so arising and including, but not limited to injuries arising from the negligence of the TSVL, its players, captains, officials, coaches, volunteers, and executive members which hereto may have been or may hereafter be sustained by me in consequence of my participation in the volleyball games, volleyball practices or social activities organized by the members of the TSVL. We hereby acknowledge that no warranties or conditions are made, expressed or implied, that volleyball games, volleyball practices, or social events sponsored by the TSVL has been, are, or will be conducted so as to prevent or minimize the risk of personal injury, and I further acknowledge that the TSVL and its executive members makes no representation whatsoever as to the competence or ability of its players to participate in the league activities in a safe manner. I further acknowledge that I voluntarily assume all risk of personal injury from participation in volleyball games, volleyball practices, or social activities organized by members of the TSVL, and that I have taken appropriate measures to make myself aware of all risks involved in the performance of such volleyball games, volleyball practices, or social activities prior to signing this Waiver. I fully understand, having read the above, that the nature and effect of this document is to release the TSVL, its players, captains, officials, coaches, volunteers, and executive members from all liability, and I hereby acknowledge my intent to release the TSVL, its players, captains, officials, coaches, volunteers, and executive members from all liability by affixing my signatures to this Waiver. Membership fees ($20 per season) are entirely non-refundable. Divisional fees ($50 per season) are also non-refundable, unless a registrant provides the TSVL executive with written notice that they wish to withdraw from participating in the league within (or before) the first two weeks of scheduled play, in which case divisional fees will be refunded.
